Living My Life Service

Living My Life Service

Living My Life disability support services are designed to support clients with significant impairment, complex needs or disabilities, resulting from injury.
 
Funded by Accident Compensation Corporation (ACC), the purpose of the Living My Life Service is to enable you to choose: where you live, what you do, where you go and who you go with.

Living My Life Services

RDNS NZ offers three Living My Life services: Facilitated Pathway Mapping; Independent Facilitation; and Tailored Supports.

These services are designed to work with you to:

  • Make choices about your support
  • Get involved in your community
  • Set meaningful goals and plan how to achieve them         
  • Build skills to live independently
  • Develop your employment skills and opportunities

When personalising these services, we can meet you in a space that is comfortable for you, whether that is in your home or at one of our designated creative spaces.

Facilitated Pathway Mapping

We meet with you to get to know you and we support you to create a visual map of what you would like to achieve and how you want to live your life.

You can use your map when you’re talking to others (including ACC, providers and community groups) to help them understand what is important to you, your family/whānau.  

We provide the creative tools; you provide the aspiration.

Independent Facilitation

We can help you to make decisions about what support you need by finding out who in your local community 'can provide them'.

We will discuss what local services you may want to become involved with, whether they are social, leisure, vocational or educational.

Once we have an idea of what you want, we arrange appointments and connect you with your chosen services.

It's your life and your decisions, we are there to guide and facilitate the process.

Tailored Supports

This service supports you to achieve what is important to you in your everyday life and they can be individual, with your friends or within your community.

Tailored Supports are designed to give you the opportunity to engage and become involved in things that are meaningful for you.

We will match you with a facilitator, who will support you to carry out your plans. This could be going for a coffee with a friend, or developing skills that help you lead a more independent life.

Living My Life Service is funded by Accident Compensation Corporation (ACC).
 
Talk to your ACC Case Manager and ask for RDNS NZ to be your provider of choice.
 
This service is available nationally across Aotearoa/New Zealand.

If you have a child with support needs in the community, including those who are medically fragile and those who live with the consequences of injury, we are here to help.
