Our range of government funded and private services includes: personal care, home help, respite care, and social support all under our restorative model of care.
 
RDNS NZ accepts referrals from a range of health and home support professionals, including general practitioners, hospitals and other healthcare providers.  

Our model of care

At RDNS NZ, we believe it is important that we support our clients to be as independent as they can in order to maintain a healthy mind and body. That is why all of our staff are trained in the RDNS Restorative Model of Care.  
 
Under this model, we work alongside our clients to remain independent. We develop a plan together, focussing on what they want to achieve. Kei tō taha ‘by your side’.

Step 1 – Refer a person to us

When you refer a person to RDNS NZ, please provide a brief overview of their medical history and or diagnosis.   
Call our dedicated Customer Services team on 0800 736 769 or send us an email referrals@rdns.org.nz or by fax to 09 589 1081.

Step 2 – We visit the person at their home

Once we have received the referral, one of our friendly clinical coordinators will arrange to visit your person at home.

Step 3 – We develop a support plan

During our visit, we work with your person to develop their individual support plan which are unique to their goals.
Your person may choose to involve you, other family/whānau, or other health professionals.

Step 4 – Person meets their support worker

Once a personalised plan is in place, and their services are arranged, your person will meet their new support worker at home.

Step 5 – Review goals and support plan

RDNS NZ will regularly monitor and review your person’s goals and progress.
We will be kei tō taha ‘by your side’ every step of the way.
