What is living My Life?

The purpose of the Living My Life service is to enable you to choose:

  • Where I live

  • What I do

  • Where I go

  • Who I go with

For ACC , RDNS NZ offers three Living My Life services. These are: Facilitated Pathway Mapping; Independent Facilitation; and Tailored Supports. 
These services are designed to work with you to:

  • Make choices about your support

  • Get involved in your community and your culture

  • Set meaningful goals and plan how to achieve them         

  • Build skills to live independently                                          

  • Build your employment skills and opportunities.

Facilitated Pathway Mapping

What is this

We support you to create a visual map of what you would like to achieve and how you want to live your life.  This map will be as big or small as you want it to be and is created in a way you believe represents you. 
You can use your Map when you’re talking to other people (including ACC, providers and community groups) to help them understand what is important to you, your family and whanau.

How do we do it?

We meet with you to get to know you, and to understand your ideas of how you want your map to look. 
We then schedule appointments with you over several weeks to develop the map.   We can come to a place you feel comfortable to achieve this work, or you can come to us in our designated creative space.  

We provide the creative tools; you provide the aspiration

Independent Facilitation

What is this?

Independent Facilitation supports you to make decisions about what support you need and find out who can provide it and where they are in your local community.

We support you to find and understand services you may wish to access in your local and surrounding community.  The decision on what, where and when to access to these services is yours.

How do we do it?

We meet with you to get to know you and discuss what local services you may want to become involved with.  These services could be social, leisure, vocational, educational - whatever you feel is required. We work with you to understand your aspirations and source suitable resources.

We can meet at a place you feel comfortable to do this work or you can come to us at our designated space. Once we have an idea of what you want we arrange appointments connect you with your chosen services.

It’s your life and your decisions, we are there to guide and facilitate the process

Tailored Supports

What is this?

This service supports you to achieve what is important to you in your everyday life.

Tailored Supports can be individual, with your friends or within your chosen community. They are decided by you and planned with you to do what is important to you. This service is designed to give you the opportunity to engage and become involved in things that are meaningful to you.                                                                                                                                               

How do we do it?

We meet with you to get to know your likes and dislikes and what it is you want us to support you with.  Once we have a plan of action we will match you with a facilitator, who will support you to take part in what you have decided and planned.  This could be as quick as going for a coffee with a friend to developing skills to lead a more independent life.
