We visit you at home We will discuss and arrange with you a suitable time for one of our team to visit you at home. We will listen to you and discuss the support you require. We develop a support plan After establishing your needs and goals, we will develop a support plan with you. We can work with your family/whānau and healthcare providers to do this. Meet your Support Worker RDNS will then arrange a support worker who we think is most suited to assist you with your needs, and can help you achieve your goals. Monitoring your goals We will monitor your support plan to make sure we are always helping you reach your goals and optimise your independence.
