We WE VISIT YOUR PATIENT AT HOME After receiving the referral, one of our friendly Registered Health Professionals will visit your patient for an assessment of their needs. WE DEVELOP A SUPPORT PLAN Together with your patient, we will develop a support plan to meet their needs and goals. They may choose to involve you, other health professionals or family/whānau in this process. YOUR PATIENT MEETS THEIR SUPPORT WORKER With a personalised plan in place and their services arranged, your patient will meet their new support worker at home. REVIEW GOALS AND SUPPORT PLAN We will monitor and review your patient's goals and progress on a regular basis and keep you informed of any changes as they arise.
