Our Approach

Your individual needs are our number one concern

By caring for you or a family member in your home, RDNS New Zealand offers you greater control, independence and choice concerning your health and wellbeing.

There are many benefits to accessing our services at home. For example, you may be able to return home sooner after a hospital stay, or stay closer to family and friends while receiving the care you need. It might mean you can regain your health in a familiar environment. After all, there is no place like home.

All RDNS New Zealand staff members are trained to provide services using the 'restorative model of care'. This means we have a strong focus on supporting each client to set and reach their personal goals. It also means that, if you choose, we will work in close contact with your doctor, community carer and family/whānau to make sure you receive the best support for you.

Our Model of Care

At RDNS New Zealand, we believe it is important that our clients strive to be as independent as they can in order to maintain a healthy mind and body. That is why all of our staff are trained in the Restorative Model of Care.

Our Restorative Model of Care is designed to encourage the client to have a say in their support services and to have an active role in deciding how RDNS New Zealand can best provide care and support to them. Under this model, we maximise our client's independence by focussing on their needs, personal goals and ongoing progress assessments.

Refer a patient to us

Call 0800 736 769 or submit a referral form by email to referrals@rdns.org.nz or by fax to 09 589 1081.

We visit your patient at home

After receiving the referral, one of our friendly clinical coordinators will arrange to visit your patient at home for an assessment of their needs.

We develop a care and support plan

Following our visit, we work with your patient to develop the best care and support plan for them. Your patient may choose to involve you, other health professionals or loved ones in this process.

Your patient meets their support worker

With a personalised plan in place and their services arranged, your patient will meet their new support worker at home.

Review goals and care plan

RDNS New Zealand will regularly monitor and review your patient's goals and progress. We will keep you informed at every step of your patient's journey with us.

Regular assessment

Your goals are our goals

To ensure your healthcare needs are met, we will assess your health needs and your personal goals. This occurs once we have received a referral.

We will work with you to develop and achieve your goals. Together with you, we’ll develop a care and support plan that best suits your needs. We will also review your goals on a regular basis to ensure you are always getting the care that is right for your needs.
