What services does RDNS NZ provide?

We offer a range of general and specialist healthcare and support services to people of all ages, in the comfort of their home. These services include:

Registered nurse services

  • Care management assessments for Home Based Support Services
  • Checks for general wellness, blood pressure, blood sugar
  • Wound care

Occupational therapist

  • Care management assessments for Home Based Support Services
  • Home assessments

Community carer (support worker) services

  • Personal care – showering, toileting, medication management, meal preparation
  • Domestic assistance – vacuuming, bed making, bathroom cleaning, shopping
  • Restorative care – assistance with exercise programs, sit / stand exercises, walking, communication / coaching to encourage clients to be as independent as they possibly can be.
Where can I access RDNS NZ services?

We believe that one of the best places to get well and remain independent is in the comfort and privacy of your own home. We work in clients’ homes and local communities. If you require a visit at your workplace or school, we can also arrange this. Most of our initial client assessments are performed in hospital so you can have access to our services as soon as you arrive home.

RDNS NZ offers care and support to clients based in the Auckland region, under the ADHB contract. The region covers Counties Manukau, Auckland, West Auckland and North Shore.

When can RDNS NZ visit my home?

RDNS NZ staff are available to speak with you and to visit 24-hours a day, every day of the year. Evening and weekend visits are available and are prioritized according to your circumstances and needs.

Within 24-hours of receiving your referral we will contact you to arrange a time to meet with you and your family / whanau. One of our Care Managers (a registered health professional) will arrange a time and day that is suitable for the initial visit. From here, we can discuss and develop a plan to best meet your needs.

Can my family be involved in my care?

Yes of course. We place a great deal of importance on working with families / whanau, other caregivers and local communities. We all need to work together to help meet your personal health goals in your home. We believe valued partnerships enhance care.

How can I receive ADHB Home Based Support Services?

The Access Centre based at the Greenlane Hospital manages all of the referrals for Home Based Support Services. Referrals are mainly made by Doctors and Hospital Discharge Co-ordinators, however anyone can make a referral to the Access Centre by:

  • Calling: (09) 307 4949 and requesting to be through to the Access Centre
  • Faxing: (09) 623 6461

Or you can contact your General Practitioner and ask him / her to make a referral on your behalf.

Can I choose RDNS NZ to provide my ADHB Home Based Support Services?

Yes. If you would like RDNS NZ to be your provider simply advise your Doctor or contact the access centre to request us.

What is the Restorative Model of Care?

At RDNS NZ we believe that to maintain an healthy mind and body it is important that we all strive to be as independent as possible. This means that RDNS NZ bases our framework of care on focusing on client needs not wants.

We will encourage you to be the best you can be by encouraging you to do as much for yourself as possible as long as it is safe for you to do so whilst encouraging family / whanau support along the way.

Our ultimate goal for you is independence but if this is unrealistic then we will work with you to be as independent as you can be, for example:

  • you may be able to wash your upper body so we will encourage you to wash your upper body and we will bathe your lower body, or
  • you or your loved one is bed bound but he / she can sit up in the bed or turn from side to side – then we would encourage you to be as active as you can be.
Are your services kept confidential?

Yes. RDNS NZ respects your right to confidentiality at all times. Our staff will not discuss your personal information with any other person unless you give them permission to do so. RDNS NZ complies with the privacy and process requirements of the Health Information Privacy Code 1994.

Does RDNS NZ provide specialist equipment and supplies that I might need?

Specific equipment and supplies are often needed to provide safe and effective home healthcare. Your Care Manager will advise you about equipment and supplies and make the necessary referrals for these to be accessible at home.

What if I am not happy or have a complaint about my experience with RDNS NZ services?

We make every effort to provide care of an excellent standard. However, sometimes things can go astray, and we value your feedback so we can continue to provide you with optimal healthcare. If you have a compliment, complaint or feedback about our services we would like to hear from you.
